- Current medications or a list of current medications and dosages
- List of any allergies you have to food or medications
- Insurance identification/membership card
- Employer insurance forms, as appropriate
- Copy of your Advance Directives
- Medical forms given to you by your Physician

Information for our Maternity Patients
As a maternity patient you will have special needs. To make your stay with us more comfortable, you may wish to bring:
- Support Bra/Underwear
- Brush and comb
- Disposable underwear and sanitary pads
- Nursing gown
- One T-Shirt for your newborn
- Cloth diapers
- Loose fitting clothes for yourself
- Clothes and blanket for your newborn
- Cloth diapers for your newborn
- Car seat
- Diaper bag
- 2 pair of disposable underwear and sanitary pads
- Infant T-shirt
